The person named herein has successfully completed the certified training listed,
by an authorized MSA instructor. This individual is certified to perform only the
duties for which they received training. This certificate is valid while employed by
the organization named, and only when the proper procedures specified in the
training, regulatory standards, and current manufacturer guidelines are followed.
This certificate is non-transferable.
